A Plan of Safe Care (POSC) is a document and roadmap developed by a pregnant woman with a substance use disorder (SUD), their healthcare provider, and a multi-disciplinary team to ensure the best possible outcome for mother and child. This can be developed voluntarily before birth of the child, and is sometimes required after birth in the case of the child being affected by substances.
Crawford County’s Plan of Safe Care Team is composed of professionals from our local health system, and several Crawford County and State Human Services agencies.
Participating Organizations include:
Crawford County Drug & Alcohol
Meadville Medical Center
